Meeting notes template: 4 formats you can copy

The best meeting note format puts action items and decisions first, then the discussion that explains them. Below are four copy-and-paste meeting notes templates: action items first, agenda-based, Cornell-style and a five-line quick recap.

Each one works as a meeting minute notes template for everyday team meetings. For formal board or committee minutes with motions and votes, use the meeting minutes template.

Which meeting note format should you use?

Format Best for Strength
Action items firstWeekly team meetings, project check-ins, client callsReaders see their tasks in seconds
Agenda-basedPlanning meetings, reviews, anything with a set agendaKeeps the meeting and the notes on track
Cornell-styleTrainings, briefings, lectures, anything you'll review laterTurns notes into a study and review tool
Quick recapShort calls, stand-ups, a recap in chatDone in two minutes

1. Action-items-first meeting notes template

This is the format most teams should default to. Write action items at the top as checkboxes, each with one owner and a due date, so the notes double as a to-do list. Decisions come next, then only the key points that someone who missed the meeting needs.

Action items first
MEETING NOTES: [Meeting name]
[Date] | [Time] | [Where / link]
Attendees: [Names]

ACTION ITEMS
[ ] [Task] - [Owner] - due [date]
[ ] [Task] - [Owner] - due [date]
[ ] [Task] - [Owner] - due [date]

DECISIONS
- [What was decided]
- [What was decided]

KEY POINTS
- [Important update, number or risk]
- [Important update, number or risk]

OPEN QUESTIONS
- [Question] -> [who will answer]

NEXT MEETING: [Date / time]

How to fill it in: leave the action items section empty when the meeting starts and add to it every time someone says "I'll..." or "Can you...". In the last two minutes, read the list aloud to confirm owners and dates.

2. Agenda-based meeting notes template

Paste the agenda into this template before the meeting so each topic already has a heading, an owner and a time box. During the meeting you only add bullet points and an outcome under each item. Every topic should end with an outcome, even if it is "no change" or "moved to next week".

Agenda-based notes
MEETING NOTES: [Meeting name]
Date: [Date] Time: [Start - End]
Facilitator: [Name] Note taker: [Name]
Attendees: [Names]

GOAL OF THIS MEETING
[One sentence]

AGENDA ITEM 1: [Topic] ([owner], [minutes])
Notes:
- [Point]
- [Point]
Outcome: [Decision / next step / no change]

AGENDA ITEM 2: [Topic] ([owner], [minutes])
Notes:
- [Point]
Outcome: [...]

AGENDA ITEM 3: [Topic] ([owner], [minutes])
Notes:
- [Point]
Outcome: [...]

ACTION ITEMS
- [Task] - [Owner] - [Due]
- [Task] - [Owner] - [Due]

PARKING LOT (for another meeting)
- [Topic]

How to fill it in: the parking lot holds topics that come up but don't belong in this meeting. Writing them down lets you move on politely without losing them.

3. Cornell-style meeting notes template

The Cornell method was designed for students, and it works just as well for trainings and long briefings. Take notes in the right column during the meeting. Afterwards, add questions or keywords in the left column and write a short summary at the bottom. Later, cover the notes column and use the cues to test what you remember.

Cornell-style notes
CORNELL MEETING NOTES
Meeting: [Name] Date: [Date]
-------------------------------------------------------------------
CUES / QUESTIONS | NOTES
(fill in after) | (write during the meeting)
                        |
[Why is launch late?] | - [Legal review needs one more week]
                        | - [Pricing page is the blocker]
                        |
[Who owns redirects?] | - [Marcus, by the 20th]
                        |
[Budget impact?] | - [None expected, confirm with finance]
                        |
-------------------------------------------------------------------
SUMMARY (2-3 sentences, write right after the meeting)
[What happened, what was decided, what you need to do.]

Tip: in Word or Google Docs, use a two-column table for the cue and notes columns and a one-cell row underneath for the summary.

4. Quick recap template (five lines)

For a 15-minute call or a stand-up, a full template is overkill. These five lines fit in a chat message or an email and answer everything people actually ask afterwards.

Quick recap
[Date] - [Meeting name] - [Attendees]

Why we met: [one line]
What we decided: [one line]
What happens next: [who] will [do what] by [when]
Anything blocked: [one line or "nothing"]

Tips for any meeting note format

  • One owner per task. "Marketing will look into it" means nobody will. Write a person's name.
  • Dates, not "soon". If the owner can't commit to a date, write "date by Friday" as the task.
  • Bullets, not sentences. Fragments are faster to write and faster to read.
  • Write decisions as statements. "Launch is April 15" beats "we talked about pushing the launch".
  • Send within a day. Notes lose most of their value after 24 hours.
  • Keep the same format every week so people know exactly where to look.

Stop choosing between listening and writing

The hardest part of taking notes is that you can't fully take part in the meeting while you write. Recording the meeting (with everyone's consent, since recording laws vary) means you can jot only the important bits and fill in your template from the transcript afterwards.

Hark is a free desktop app for Windows, Mac and Linux that records your microphone and, on Windows and Linux, your computer's audio, so it works with Zoom, Teams, Google Meet or any other call. It transcribes on your computer (English only), labels who spoke, and has a notes editor next to the transcript for your own notes. On a Mac, Hark records the microphone only. With a subscription or the 2-day preview, Claude can write AI meeting notes in a choice of styles, and you can add your own style file, such as one of the formats above. Frequently asked questions

What is the best format for meeting notes? +

For most work meetings, an action-items-first format works best: action items with owners and dates at the top, then decisions, then key discussion points. People who missed the meeting can see what they need to do in seconds. Use an agenda-based format for longer structured meetings, and Cornell-style notes when you need to study or review the material later.

What should be included in meeting notes? +

At minimum: the meeting name, date and attendees, the decisions made, action items with one owner and a due date each, and any open questions. Add short discussion points only where they explain a decision. Leave out word-for-word dialogue.

What is the difference between meeting notes and meeting minutes? +

Meeting minutes are the formal, approved record of an official meeting such as a board or committee, with motions and votes. Meeting notes are informal and written for the team, usually focused on decisions and next steps. Minutes follow a set structure; notes can use whatever format helps.

What are Cornell notes for meetings? +

Cornell notes split the page into a wide notes column, a narrow cue column and a summary box at the bottom. You write notes during the meeting, then add questions or keywords in the cue column and a two- or three-sentence summary afterwards. It is a study method that also works well for training sessions and long briefings.

How do I take meeting notes faster? +

Prepare the template with the agenda before the meeting, use short bullet points instead of sentences, and write action items the moment they come up. Many people also record the meeting (with everyone's consent) so they can listen to the meeting and check details later instead of writing everything down.
